An important notice from Sarah Perlman

Sarah Perlman, CNP, will have her last day seeing patients in office on October 21, 2024 at the location of 3421 Farm Bank Way in Grove City.

 

Sarah is moving to Washington state to be close to her family.

She has this statement to share:

I appreciate the trust and support serving as a primary care nurse practitioner within central Ohio and will continue to bridge the transition with much of the same office staff. You can still contact me through the Athena portal or through our regular phone 614-878-7285 during the transition. We are still making appointments for our office.

We assure you that your medical records will remain confidential. If you have any questions or concerns, please don’t hesitate to contact our office. We will be happy to assist you in any way we can.

Annual Wellness Visits

Protecting Your Health For Years To Come

The Annual Wellness Visit (AWV) is a yearly appointment with your primary care provider (PCP) to create or update a personalized prevention plan. This plan may help prevent illness based on your current health and risk factors. Keep in mind that the AWV is not a head-to-toe physical. Everyone enrolled in Original Medicare and Medicare Advantage, though, is eligible for an annual wellness visit for no copay. Schedule yours today!

Sarah Perlman

Owner/Family Nurse Practitioner

Sarah is a 2011 graduate of The Ohio State University with the Family Nurse Practitioner (FNP) specialty track.

 

After working for Dr. Paul Guitheil for several years, Sarah purchased the practice to continue providing quality, independent primary care to Central Ohio West Side communities, including Grove City.

Sarah enjoys stand-up paddleboarding, visiting her son at the University of Cincinnati and finding new, vegetarian-friendly restaurants.

Stacy Whisner

Family Nurse Practitioner

Stacy is originally from a small town in West Virginia.  She obtained her Bachelor's degree in Nursing in 1995 from Alderson-Broaddus College.  She graduated in 2000 with her Masters in Nursing as a Family Nurse Practitioner from Marshall University. 

She moved to Columbus in 2003.  She has had a variety of experiences including Family Practice, Oncology and Addiction Medicine.

 

Stacy enjoys spending time with her husband and son.  She also enjoys doing outdoor activities including kayaking and hiking.
